Adult Foster Care (AFC) is a state-funded program designed to allow for seniors and persons with disabilities who need assistance with personal care tasks to remain in their home and in the community with a qualified caregiver.
The Adult Foster Care (AFC) program is responsible for overseeing and paying caregivers who provide care to individuals in a qualified home. If you are a caregiver who provides physical assistance or supervision, More Care AFC offers a comprehensive solution.
This is a service provided at no cost to members of MassHealth. More Care AFC offers comprehensive caregiver training and resources for a wide range of health conditions, including:

* Neurological disorders (Stroke, Parkinson’s disease, Multiple sclerosis, Alzheimer’s disease)
* Heart Disease
* Obesity-related complications
* Generalized weakness and functional decline
