Adult Foster Care

As a member of our Adult Foster Care (AFC) program, you will live with a trained caregiver, who may be a family member or a trusted family friend. Your caregiver will assist with daily activities such as bathing, dressing, toileting, transferring, walking, and eating, while also offering transportation for medical appointments and companionship.
Our involvement ensures participant safety through a collaborative approach. Our multidisciplinary care team—consisting of the participant, caregiver, registered nurse, and case manager—works together to develop a comprehensive care plan that addresses medical, physical, emotional, and social needs.
We provide invaluable support to participants and caregivers. Our registered nurses and case managers often serve as the primary point of contact, helping to relieve stress and prevent caregiver burnout.

Member Benefits

Your Caregiver, who can be a family or non-family member, will:

  • Prepare meals and snacks.
  • Assist you will personally care (bathing, dressing, toileting, transferring, ambulating, and eating).
  • Provide medication reminders and assistance.
  • Do shopping, laundry, and housekeeping.
  • Provide transportation to medical appointments and social visits.
  • Maintain a clean and safe living space.

Eligibility Requirements

To qualify for Adult Foster Care, the following requirements must be met:

  • Potential members must be over the age of 16, with MassHealth
  • Potential members & authorized caregivers MUST have a qualified home setting.
  • Potential members must need assistance with ADLs such as bathing, dressing, feeding, transfers and walking. 
  • Potential caregivers must be over the age of 18.

Refer a Caregiver or Family in Need

At More Care AFC, we welcome individuals with diverse experience in supporting older adults and those with disabilities or unique challenges. If you know someone who shares our commitment to ensuring that all individuals in need have received personalized, in-home services and support-they may qualify to become a caregiver or a recipient within our Adult Foster Care program.

Receive $300 when you refer someone who may be interested in becoming a caregiver with More Care AFC.
